One of my greatest weaknesses in writing is to take leaps of thought -- but the same is true of many writers. Imagine following a person's footprints in the sand. Suddenly they take a giant leap. You are disorientated while you look for where their footprints came down again. Typically it is not an error of thought, but it is a wont which is no good for coherence.
OBSERVATION: Apart from this, one of the biggest omissions I find in editing other people's work is too-abrupt introductions. This can make a big difference.
